We are located at: 245 Eglinton Avenue East, Toronto, ON. M4P 3B7
Clinic access is via our street entrance on Eglinton Avenue East and NOT through the communal area of the building in which we are located.
We kindly request that clinic users DO NOT enter the communal area. This area is not linked to our service and any accidents that occur in this area WILL NOT be covered by the clinic.

If you have been to our clinic previously, please refer to our registered patients page.
If you are a new patient, you need to fill in a "new patient" form on our website. We will call you as soon as possible to schedule your first appointment with a new doctor.

If you require urgent medical attention outside of clinic open hours that cannot wait until the next available routine appointment do call the clinic on 437-826-3005. These clinics run Mon-Thur 5-8pm and a there will be a 3-hour clinic on Saturday or Sunday morning. We aim to publish a calendar of availability here ASAP. Please note that doctors will only be able to address one issue per consult as this is for urgent on the day matters only.
Please ensure we hold details of a valid healthcard number to prevent unexpected fees.

Please contact us at least 24 hours before your appointment by dropping us an email at admin@mpclinic.ca or by calling us on 437-826-3005.
If you miss your appointment without letting us know, you will be charged $60. This charge may also be applied if you are late to your scheduled appointment.

Once you have been allocated to a physician that is accepting new patients and have attended a 'meet and greet', it is our policy NOT to allow switches to other physicians within the clinic, for the following reasons:
a. Our physicians are fully qualified to provide primary care to patients of all ages and genders.
b. Our physicians work as a collaborative team, and are therefore hesitant to contradict a colleague if a conflict arises between a patient's request and their physician's professional opinion or advice. Patients deserve an unbiased second opinion, which can only be obtained through a new physician in a different clinic.
c. Taking on new patients requires a significant amount of administrative work to review health history as well as old medical records; allowing switching would increase this administrative work, thus reducing physician availability for medical care.
d. As our physicians work in a collaborative group practice, if you are unhappy with your current physician, there may be instances where you would have to see that physician again should you stay with another physician in the clinic; this has the potential to result in sub-optimal medical care.
This policy does not prevent patients from reasonable access to a physician; we are fortunate to be located in Toronto with a high density of health care providers, and patients may certainly seek care with a new physician at a different clinic. -
There is limited street parking near the clinic.
The nearest Green P parking can be found at 650 Mt Pleasant Rd, Toronto, ON M4S 2N5.

‘No news is good news’
Our reception team do not give test results over the telephone unless authorised by clinical personnel.
Patients with ‘abnormal’ results will be notified.

Prescriptions created during a consultation with a physician or nurse practitioner are free to the patient via OHIP.
Prescription renewals generated outside of a consultation incur a fee of $30 per medication as this is not covered by OHIP.
Renewals of prescriptions usually require a visit to the clinic to re-assess the condition requiring the prescription.
Please anticipate your renewals, ask for renewals at each visit and bring your medications to your appointments.

Our staff have the right to be treated with dignity and respect at all times without the risk of threatening and/or violent behaviour.
Abusive behaviour will not be tolerated towards our staff and may result in offenders being asked to leave and could be removed from our patient list.
In extreme cases police will be called and legal action taken.
